  • Overview

    Responsible for caring for vulnerable patients enrolled through AccessHealth or identified by EMS or allied health partners in their home or other community setting.  Participates in planning of programs or objectives for own work group.


    Examines, screens, treats and coordinates health services for patients.  Conducts post hospital or clinic release follow-up care including, but not limited to, monitoring medication, dressing changes, and checking vital signs.  Follows physician orders for mobile, at site patient care visitations in home or other residence facility; Administers patient care consistent with department protocols.  Observes, records, and reports to physician, patient’s conditions and reactions to drugs, treatments and significant incidents.  Conducts patient education, including diabetes prevention/treatment, Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), falls assessment, injury evaluation, geriatric/vulnerable patient visits and nutrition.  Coordinates appointments and follow-up with physicians and hospitals/clinics.  Develops and completes appropriate reports for the Community Paramedic Program; Attends meetings as requested and available.  Performs all primary job duties of a Paramedic II.  Participates in activities and duties related to emergency management during a local state of disaster as directed by appropriate county managers.


    High School Diploma/GED; State of Texas Health Services certified or license paramedic.  Three years of pre-hospital experience, one year as a Paramedic II with Fort Bend County or an equivalent pre-hospital EMS service.  Strong verbal and written communication skills, and strong organizational skills, strong interpersonal skills and ability to deal effectively with the public and other employees.  Ability to complete projects.  Completion of Community Paramedic orientation training including the Community Health Worker certification within three months of employment.  Subject to 24-hour emergency recall.  Schedule requirements may involve working nights and weekends.  Valid Driver’s License.  This position is Grant Funded.


    SALARY RANGE: Public Health Grade 11, $2,057.60 - $2,420.00 biweekly based on qualifications
